Detailed Overview

Reporting administratively to the Manager, Clinical Services and clinically to the Professional Practice Chief, Social Work, the Social Worker, Residential Services uses professional and program standards in providing psycho-social assessment, crisis intervention counselling and specific resident treatment, as well as assisting with tasks that are a cross-responsibility of all team members. Responsibilities Facilitates all admissions to the facility by liaising with Access, Home Health and clients; conducts admission interviews and tours; issues medical forms; completes required documentation; schedules admissions and conducts admission conferences for staff. Assists residents and family members/significant others with financial matters including equipment acquisition, government applications and other matters as appropriate.
