Use best-practice and innovative approaches to provide client-centered services including assessment, treatment and psycho-social rehabilitation.
Regularly update client service plans (including crisis plans).
Provide direct assistance and support to clients regarding housing, employment, relationships & leisure activities, activities of daily living, and transportation.
Work collaboratively, cooperatively and effectively within the FACTT DD team and with other community-based agencies including hospital inpatient units and emergency.
Demonstrate thorough knowledge of the theories and practices of a professional social worker as they relate to family, individual and systems interventions.
