The Mental Health Nurse will be a member of the interprofessional team providing assessment, care, and treatment in Psychiatry. In collaboration with other team members, the Nurse will:
Provide psychosocial support and counselling, support patients through regular, patient-centred follow up.
Identify appropriate resources to patient needs taking into consideration severity of illness, culture, language, reading level and health literacy.
Taking into account each patient’s values, preferences and circumstances, support the development of patient-centred goals.
Facilitate family team meetings as needed.
Implement interventions/ treatment plans with patients, family members, significant others, and the multi-disciplinary team for the purpose of facilitating changes in behavior, attitudes and feelings.
