Provides support and/or assistance to the residents to meet their needs and expectations as per the residents’ individual plan of care in the activities of daily living, including but not limited to personal hygiene, mobility, toileting, and eating.
Provides support by maintaining the cleanliness and neatness of resident living area, ensuring the resident’s equipment is safe and functional.
Accurately measures weight, height, intake and output and documents on Point of Care.
Accurately measures Vital signs as delegated by the team Leader on the home area.
Communicates all changes in resident status to the Team Leader and accurately completes documentation on POC to reflect the care delivered.
Improves the residents’ optimum functional ability by providing restorative care processes in the specific personal care area using assistive and adaptive measures as ordered.
