Patient Intake Coordinator ** On-site in Kalihi ** Overview We are seeking detail-oriented and customer-focused Patient Intake Coordinators to support a fast-paced healthcare team. In this role, you will serve as the first point of contact for new patients, helping coordinate care and ensuring a smooth onboarding and scheduling experience. This position is ideal for individuals who enjoy working in a high-volume call environment, managing multiple tasks, and delivering excellent service. Key Responsibilities
+ Receive and review new patient referrals from hospitals and clinics
+ Conduct outbound outreach to patients to coordinate dialysis care and scheduling
+ Handle inbound calls from patients following up on scheduling requests (average 20/day)
+ Manage patient cases through a Salesforce ticketing system
+ Track and follow up on open cases, including patients who have not responded
