Providing expert clinical care to hospice residents; expert clinical advice to healthcare clinicians at hospice and to nurses on the role of pain/symptom management, psychosocial supports & therapeutic palliative care interventions.
Participate in resident rounds and case conferences with the care team routinely & as needed.
Work collaboratively with palliative care physician team to assess and manage patients related to their medical care, pain/symptom management, psychosocial assessments, counsel caregivers & residents and to lead prevention initiatives.
Collaborate and consult with in-house palliative care clinicians but also external care partners including hospitals, palliative care units, hospices, home care and other community agencies and healthcare providers.
Provide advice regarding resident care issues and consult with others when needed.
