The practice consists of six Nurse Practitioners supporting longitudinal primary care. The clinic is a Primary Care Network partner, with Allied Health providers co-located, working alongside Primary Care Providers supporting clinic patients.

This leave is FT; locum interest for partial coverage also accepted.

Start date: ASAP for one month; possible extension pending return of incumbent.
