Job Title: Medical Coordinator – Street Health & Supported Housing

Reporting to the Medical Director of Homelessness, Supported Housing and Complex Care in Vancouver Community, the Medical Coordinator, Street Health and Supported Housing provides physician leadership across four interdisciplinary teams (MoDe, ICRT, INO, IHOT) and designated supported housing sites, including Victory House. The teams are as follows:

• Intensive Health Outreach (IHOT) team – provides low‑barrier interim health care, and psychosocial support for unhoused/vulnerably housed people in Vancouver. With an ultra‑low barrier clinic that runs on Mondays, Wednesdays and Thursdays at the Molson Overdose Prevention Site (across from InSite)
