The Hospital to Home (H2H) program is a short‑term care initiative designed to help patients transition from hospital to home, particularly when they no longer require acute care but need extra support. The goal is to support timely discharge from hospital to reduce awaiting long‑term care (ALC) and reduce 30‑day readmission rates.
As a member of the care team in the transition program, this role participates as part of the multidisciplinary team with the provision of patient care in hospital and community. The Registered Practical Nurse provides patient care under the direction of the care team and/or physicians in accordance with hospital standards, policies and procedures.
