Consult with and advise the Food Service Manager and Director of Care concerning nutrition and hydration program needs; responsible for nutritional assessment, care planning, implementation, and follow-up for residents.
Complete nutritional assessments for each resident using RAI MDS 2.0 as required; determine an individualized resident’s diet and other nutritional interventions, involving the resident, family members and care team.
