Detailed Overview

Works as a member of the Home Health interdisciplinary team to provide assessment and treatment services for clients; liaises with community agencies and caregiver support groups; participates in educational services, planning; program development and policy recommendations; provides consultative support to the interdisciplinary team in the development of services within the assigned area of responsibility. Responsibilities Provides therapeutic counselling services to clients and families experiencing complicated circumstances related to a variety of medical diagnoses.
