Facilitates an interdisciplinary approach to chronic disease management and self-management support as per the Ontario Chronic Disease Management Framework.
Provides direct, expert, comprehensive care/nursing services, incorporating the Chronic Care Model to improve clinical outcomes for patients with diabetes, through health promotion and preventive care, empowering patients and families to use self-management methodologies to positively affect their health outcomes.
Demonstrates expert knowledge and synthesis of advanced nursing practice to perform comprehensive health assessments and clinical decision making.
