Diabetes Educator And Primary Care Clinical Dietitian

Contract: Maternity Leave (16 months)

Full Time (1.0FTE): 3 days (0.6) in SUNDEC 2 days (0.4) in SAFHT

Sunnybrook Academic Family Health Team (SAFHT) and the Sunnybrook Diabetes Education Centre (SUNDEC) are looking for a highly motivated and self‑directed Registered Dietitian to join our dynamic interprofessional team. This is a dual role, participating in both SUNDEC, serving a complex population of patients with diabetes, as well as at the SAFHT in primary care. Care for patients with diabetes will include Type 1 and Type 2 diabetes, with many requiring insulin. Care for primary care patients at SAFHT will include helping support their cardiovascular health, metabolic management, digestive system diseases and pediatric nutritional issues.
