Care Partnerships Manager

The Care Partnerships Manager will lead the development, coordination, and maintenance of institutional partnerships across the perinatal, reproductive, and social services landscape. This role is ideal for someone experienced in inter‑organizational relationship building, health system navigation, and information systems at provincial and federal levels, who is passionate about equity, collaboration, and culturally safe care.

You will support the expansion and formalization of referral agreements, oversee operational integration of Mino Care staff within partner institutions, and ensure smooth cross‑sector collaboration aligned with the three pillars of the Perinatal Pathways Program.
