The salary range for this position is CAD $42.56/Hr. - CAD $52.95/Hr.

Job Summary

Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, responsible for coordinating a centralized referral and access utilization system for Adult, Older Adult and Youth (aged 14 – 18) clients. Provides services for Youth, Adult and Older Adult Tertiary Mental Health & Addiction Services including services to the LGBT2S (Lesbian, Gay, Bi-sexual, Transgendered, Two Spirited) youth population. Determines the effectiveness of access and discharge policies and procedures.
